Start Here

  1. Make sure that your version of Adobe Acrobat Reader DC is up to date.

  • Mac/IOS: Adobe Acrobat Reader DC is available for free on the Adobe website. 

  1. Start by downloading the Checklist: Enrollment-Contracting Checklist

  2. Navigate through the checklist requirements by downloading the forms found in their respective files in the sidebar.

  3. Once a PDF has been downloaded, open it in Adobe Acrobat Reader DC.

  • Note: Not all files will open in your web browser.
  1. Most forms are fillable, so where possible type in the information versus hand writing it.
